Killing the dog and leaving the note are both silly. Just go up to him and ask him face to face. I'm sure he'll take your request more serious and actually respect you a little more thus him taking action more likely. You leave a note hes likely to laugh at it for you being a pussy.

In all seriousness I dont think you will get very far with this because

1) it's an animal, he cant just tell it to shutup
2) he lives far enough away that he's just going to write you off as an anal tight wad who should stfu or turn on a house fan
3) the dog has grown accustomed to staying outside
4) the owner has decided to have an outside dog, so it's not like he's going to completely reverse his lifestyle and suddenly start living with one inside

So, unless he just bought this dog and it's keeping you up.. or he just starting putting the dog outside for the first time... or the dog has always lived outside and suddenly starts barking, what can anyone really do? This is just part of the shit if living in your own property.. because your neighbors also own their property and can pretty much do whatever they want. I live in an apartment and if that shit started happening you'd better believe they'd be forced to deal with it, even if the owner pulled the above 4 excuses.

Yep. Seems like the far best option. Go there and have a talk. It's perfectly reasonable to complain about matters like this. And also, there must be other in the neighborhood that is bothered as well.

I think face to face is the way forward but I think you could also consider as backup some really good earplugs - serious - I needed some a while ago and they are pretty good.
