this is one of the most obvious photoshops I have ever seen... the fingers give it away completely. nobody would even hold that piece of paper that way, the fingers are way to awkward

Looks to me like there is a picture of a girl - like a physical picture - and somebody else is holding the note on to the picture and then a photo is taken of that. I'm not sure it's shopped - it's just that if it is supposed to be the girl in the photo holding the note it's bollox.
